DSM-5 TR 개편 수정 사항-2. updated disorders 개인적 중요도 상

중요하다고 생각되는 것 먼저 다루고, 나머지는 시간이 있으면 정리해보려고 한다. 

우선 내 생각에 중요한 장애는 추가된 세 가지 장애이다.

 

(1) Suicidal Behavior and Nonsuicidal Self- Injury 

자살 행동과 자살 의도 없는 비자살적 자해

- Part II에 “other conditions that may be a focus of clinical attention"으로 코드 추가됨

- 루틴한 임상적 시행에 자살행동과 자해행동을 고려하도록 하기 위함. 향후 자살시고와 죽음의 위험도를 파악할 수 있음. 또한, (개인적으로 중요하다고 생각되는 부분) 자살/자해행동이 주요우울장애 같은 질환의 증상 하나로 보여지기보다는 특별히 다뤄져야 할 부분으로 targeting하도록 함. 

-> 청소년, 성인 초기 자해 행동이 눈에 띄게 증가했다. 한 시대가 conversion이 유행이었다면 최근은 공황과 NSSI가 유행인 듯한.. 이에 대해 추가적으로 고려하고 지속적으로 관심을 두어야 할 것 같다. 꼭 Depression으로 인한 것이 아니더라도 DBT의 고통감내기술과 같이 심리교육이나 치료도 필요할 것 같다. 

 

(2) Prolonged Grief Disorder

지속성 애도 장애

- 외상 및 스트레스 관련 장애 챕터에 코드 추가됨.  진단의 적합성, 최신 연구들을 확인하여 2009년 DSM steering commitee에서 승인되었음.

장애의 정의는 다음과 같다. 

Prolonged grief disorder is defined as intense yearning or longing for the deceased (often with intense sorrow and emotional pain), and preoccupation with thoughts or memories of the deceased (in children andadolescents, this preoccupation may focus on the circumstances of the death). 

-> 부모, 자녀, 배우자의 죽음 이후 1년 이상 죽은 이에 국한되어 우울감, 죄책감이 나타나는 경우가 있는데, 일반적인 우울과는 특성이 다르고 다른 개입이 필요하기 때문이었을 것이라는 생각이 들었다.

 

(3) Unspecified Mood Disorder

명시되지 않은 기분 장애

- DSM-5 Mood Disorder group이 삭제되면서 함께 삭제되었던 진단을 되살림.

- It will be applied to presentations that do not meet criteria for any specific mood disorder and in which it is difficult to choose between an unspecified depressive disorder and an unspecified bipolar disorder.

Mixed presentation일 때 더욱 정확하고 낙인을 적게 하는 진단을 하기 위함.

- Depressive Disorder 정도의 기능 손상이나 주관적 고통감이 있으나 depressive disorders diagnostic class, adjustment disorder with depressed mood, or adjustment disorder with mixed anxiety and depressed mood 에 해당되지 않는 경우에 해당됨. 특히 어떠한 진단에도 해당되지 않지만 정보가 충분하지 않은 응급실 상황 등에서 쓰일 수 있음. 

-> 사실상 Unipolar Depression인지 Bipolar Depression인지 잘 모를 때 사용할 진단을 되살린다는 것이다. 여전히 이 둘을 구분하는 것은 쉽지 않기 때문에 이 진단을 다시 추가하게 되었을 것으로 생각된다. 

 

이 외에는 용어 수정이 대부분인 것 같다.
